Value set

In short

A value set is the list of codes allowed for a specific data element, drawn from one or more terminologies: for example, the diagnoses accepted in a problem list or the tests valid in an order. It defines which answers are valid in each field so that every system records the same thing the same way. They are published by, among others, implementation guides and national terminology programmes.

Explanation

What it is

A value set selects codes from one or more code systems, such as SNOMED CT, LOINC or ICD, for a specific use. The code system defines the concepts; the value set says which ones can be used in a given context. FHIR takes these concepts from the HL7 v3 Core Principles, so the idea predates FHIR and does not depend on it.

Definition and expansion

A value set has two aspects:

  • Definition (compose, the "intension"): the rules that state which codes it includes.
  • Expansion (the "extension"): the list of codes it actually contains at a given time and with certain terminology versions.

Depending on how it is defined, it can be:

TypeHow it is definedBenefitCost
IntensionalWith rules, for example all subtypes of a conceptUpdates itself when the terminology adds codesThe result changes with every version
ExtensionalBy listing every codeFull control over its contentMust be kept up to date by hand

In FHIR, the $expand operation asks a server to compute the expansion, and $validate-code checks whether a code belongs to the set.

Binding to a data element

A value set is bound to an element of a data model with a strength. FHIR defines four:

  • required: the code must come from the set.
  • extensible: it must come from the set if any code applies; if none covers the concept, others are allowed.
  • preferred: using it is encouraged but not mandatory.
  • example: it only illustrates the kind of concepts expected.

Common misconceptions

  • A value set is not a terminology: it does not create concepts, it selects them.
  • The expansion of an intensional set changes when the terminology version changes. For reproducible results, pin the versions.
  • Binding strength matters: a validator rejects a code outside a "required" set, but not outside a "preferred" one.
  • Whoever publishes a value set must maintain it. An abandoned set keeps circulating with obsolete codes.

Usage examples

  1. The US National Library of Medicine's Value Set Authority Center (VSAC) hosts public value sets and provides the value sets of the CMS electronic clinical quality measures (eCQMs).

  2. The HL7 IPS guide binds the clinical problem code to the "Problems - IPS" value set, based on SNOMED CT, with preferred strength.

  3. The FHIR vital signs profile uses extensible bindings: if a system records a vital sign from the list, it must represent it with the specified LOINC code.

  4. The IHE Sharing Value Sets (SVS) profile defines how systems retrieve value sets from a central repository, for example by their OID.

  5. Australia's National Clinical Terminology Service publishes national reference sets and value sets, and runs the national terminology server.

  6. An allergy form shows only the allergens in a national value set, and the system rejects codes that do not belong to it.

Depend on this one2

  • FHIRStrong

Almost all FHIR elements with a coded data type are bound to a value set with a binding strength.

  • Terminology serverStrong

Expanding and validating value sets ($expand, $validate-code) is one of its core functions.
