IHEIntegrating the Healthcare Enterprise
In short
IHE is an international initiative of healthcare professionals and industry that turns standards such as HL7 and DICOM into concrete integration recipes, called profiles, and tests them across vendors. For anyone buying systems, it is the way to ask for interoperability by a precise name instead of a generic promise.

Explanation
What it is
Integrating the Healthcare Enterprise (IHE) is an initiative by healthcare professionals and industry to improve the way health IT systems share information. It promotes the coordinated use of established standards, such as DICOM and HL7, to address specific clinical needs. Its legal entity is IHE International, Incorporated, a non-profit organization incorporated in the State of Illinois (USA). It has more than 100 member organizations: professional societies, government agencies, provider organizations, technology companies and others.
IHE began in 1998, when HIMSS and RSNA decided to work together to improve integration between imaging and information systems. Its first successes were in radiology.
How it works
IHE follows an annual four-step cycle:
- Clinical and technical experts define critical use cases.
- Technical experts write detailed specifications, the profiles, which select and optimize existing standards.
- Industry implements those profiles in its products.
- IHE tests the systems at supervised events called Connectathons.
Each profile defines actors (the role of each system), transactions and content. Profiles are compiled into freely available Technical Frameworks. The work is organized in domains, such as IT infrastructure, radiology, patient care coordination, pathology and laboratory medicine, cardiology or pharmacy, and in national and regional deployment committees.
Common misconceptions
- IHE is not a standard. It is the organization that publishes profiles based on other bodies' standards.
- "IHE compatible" means nothing in a tender. You need to name the profile, the actor and the options, for example "XDS.b, Document Consumer actor".
- An IHE Integration Statement is published by the vendor itself; it is not a certification.
- Testing at a Connectathon is not a product certification either. IHE has a separate conformity assessment program.
Usage examples
IHE holds Connectathons every year in Asia, Europe and North America, where thousands of connections between products are tested, and publishes the results in a database.
The IHE Radiology Scheduled Workflow profile coordinates the order, the modality worklist and image storage between the radiology system and DICOM devices.
The XDS.b profile in the IT Infrastructure Technical Framework defines how to register and query clinical documents across organizations.
The MHD profile offers a FHIR interface for document sharing; it has one edition for FHIR R4 and another for R5.
The Hajj pilgrimage use case in the IHE Verifiable Health Links profile describes how pilgrims share their IPS, verified through the WHO GDHCN.
The IHE IUA (Internet User Authorization) profile defines how to authorize RESTful health transactions with OAuth access tokens.
Relation to other terms
Relies on5
- DICOMStrong
IHE was born in radiology, and its imaging profiles rely on DICOM.
- HL7 v2Strong
Many IHE profiles, especially in radiology, laboratory and IT infrastructure, specify their transactions with HL7 v2 messages.
- CDAMedium
Several IHE content profiles define CDA documents that are then shared with XDS.b.
- FHIRMedium
IHE's newer profiles, such as MHD, PIXm, PDQm or IUA, are defined on FHIR or REST APIs.
- IPSLow
IHE publishes profiles that share the IPS, such as the Patient Care Coordination IPS profile and Verifiable Health Links, but the rest of its work does not depend on it.
Depend on this one1
- OpenHIEStrong
Its exchange workflows are specified with IHE profiles and transactions, although the architecture is not limited to them.
